Online application for library cards is restricted to Florida's state agency employees. Florida residents can apply for a library card in person at the State Library.
To apply for a library card, update your registration information, or report a lost card, complete all of the fields below. Information is for internal use only. Your application will be submitted when you acknowledge that you understand and agree to the statement of responsibility included at the bottom of the form.
We will contact you via e-mail within three working days after submission of your application. If you have not received a response from the State Library of Florida within three working days, please contact Lending Services. For additional assistance, please call the State Library at 850.245.6680 and ask for the Circulation Department.
The State Library may contact you or your supervisor to confirm employment with the State of Florida.
